what is the problem that needs to be fixed? Spectator mode seems to be working fine on my servers.

I have this problem sometimes again, it's depend when you change the camera and when you go to the spectator mode. But it works better now.

what is the problem that needs to be fixed? Spectator mode seems to be working fine on my servers.

Most time when the map change the spectators are in free mode (while is still show follow or replay on bottom right).
It also happens sometimes at beginning of rounds, and also sometimes when your spec target finishes, become spec, or leave the server. But it's more to be fixed by a game update now.
