Ok, now let's check some launcher options.

In the advanced configuration - Inputs.

Is use alternative method checked?

Is freeze unused axes checked?

For both, check or unset individually and try if it changes something.

I don't know if this has changed recently or not but the in game configure controller menu, assigning a controller button by pressing it once adds it to a list of buttons for an action. Selecting the action (IE:acceleration) and pressing a button to assign and second time, for some reason removes previously assigned buttons and adds the new one.
Have you tried this?
CPU: Intel I7-4770 64Bit @ 3.4Ghz.
Ram: 32GB DDR3
GPU: Geforce GXT 1060 6.2GB DDR5
Windows: 7 Pro SP1 64bit

That's really weird.
Are you using a tool to remap the buttons by chance? If not, you might try some.

I don't know, isn't one there from Microsoft directly?
Otherwise search yourself, for xpadder etc.
