How do I play my custom made map online with people?

I made a map for shootmania. How do I play it online with people? Am I supposed to upload it to the game somehow, or is it just like quake and I have to talk to some server admin and ask him to run it on his server?

ask a server admin for uploading it on his server or host a server by your own

You can also upload your map to mania exchange
http://sm.mania-exchange.com/
most server admins get their maps from there, so if you map is good you may find it on a lot a servers.
or you could start an ingame net server, load you maps on it and wait for others to join.

port forward 2350, 2450, and 5000

I have opened these ports, but I still cant connect. Any ideas?

You need to forward them from your adsl box/router to your servers ip.
On your server's firewall they need to be open.
